Community Health Workers  

  A community health worker (CHW) is a member of a community who is chosen by community members or organizations to provide basic health and medical care within their community, and is capable of providing preventive, promotional and rehabilitation care to that community. SCAN is committed that every SCD patient and family has access to CHWs who have experience in supporting those impacted by SCD to assist as they navigate through the healthcare system and address any social barriers that can impact the health and wellbeing of SCD Warriors and their families.

CHWs can help patients and families with getting connected to community resources to establish assistance with things such as workforce development, mental health services, food , housing, transportation assistance and more.

CHWs can help you to better understand your health insurance and better utilize your valued added benefits such as case management, gym and warehouse grocery memberships, nutrition support, medical transportation, all which are coverages under your insurance that you may not currently be taking advantage of.

CHWs can be an additional supportive person in your life. If you have a question, or even just want to talk, your CHW is there to support you. Need more than the standard reminder calls about doctors appointments or prescription refills? CHWs can call, text, and email you just to make sure you don't miss your refill or appointment. 

All CHWs with CBOs have recieved HIPPA Compliance Certification and are certified as Community Health Workers through the state of Nevada.
